information comes from a variety of sources including a worksheet the mother fills out, the 
mother's prenatal record, and the delivery record. The Office of Vital Records and Statistics 
has a Quality Control program where every hospital is audited annually. Births are randomly 
selected and hospital records are checked to verify the accuracy of the reported information.</TEXT>
