      • abstract: This dataset contains case information occurring among Utah residents for 1) inpatient hospitalized and unintentional carbon monoxide poisoning and 2) inpatient and outpatient carbon monoxide poisoning emergency department encounters. The indicators in this dataset are age group, five-year age group, county, race, ethnicity, sex, number of admissions by year and month of admission, and a fire-related cause statement. The carbon monoxide poisoning data was obtained from the Utah Hospitalization Discharge Database and heat stress data was obtained from the Utah Emergency Department Encounter Database. The Utah Environmental Public Health Tracking Network received all data through the Office of Health Care Statistics in the Utah Department of Health. The data was aggregated by the Utah Environmental Public Health Tracking Network.
      • Important: These data provide summary information about the rates, patterns, and distribution of hospitalizations and emergency department visits from carbon monoxide poisoning in the state of Utah. This data is available for public use.
      • Measures:
        • Derivation:
          • Hospitalizations:
            • Numerator: Number of emergency department visits for carbon monoxide (CO) poisoning
            • Denominator: Midyear resident population

            • Denominator: Not applicable
          • MeasureUse: These data can be used to understand the distribution and patterns of hospitalizations and emergency department visits in Utah due to carbon monoxide poisoning.
      • Unit: Carbon monoxide poisoning refers to illness that results from the exposure to excess amounts of carbon monoxide, defined with the following ICD-9 (International Classification of Diseases, 9th Revision, Clinical Modification, established by the United States Center for Health Statistics) codes: Tox eff carbon monoxide 986; Acc poison-exhaust gas E8682; Acc pois-co/domestc fuel E8683; Acc pois-carbn monox NEC E8688; Acc pois-carbn monox NOS E8689; Poison-exhaust gas E9520; Undeter pois-exhaust gas E9820; Undetermin poison-co NEC E9821 [excluding E950.0-E979.9 and E990.0-E999] or IC9-10 codes: T58.01; T58.04; T58.11; T58.14; T58.2X1; T58.2X4; T58.8X1; T58.8X4; T58.91; T58.94 [excluding T58.01; T58.04; T58.11; T58.14; T58.2X1; T58.2X4; T58.8X1; T58.8X4; T58.91; T58.94].

        • supplinf: The Utah Hospitalization Discharge Database (UHDD) provides information about the primary and contributing diagnoses of individuals hospitalized in Utah. The Office of Health Care Statistics (OHCS) serves as the official repository for UHDD data per the Utah Code Annotated Chapter 33a, Title 26, Utah Administrative Rules R428-10 and R428-11, and a Memorandum of Agreement between the Utah Environmental Public Health Tracking Network (UEPHTN) and the OHCS. The OHCS collects complete, timely, and accurate emergency department case information and disposition data for all hospitalizations occurring in Utah. Before providing annual data updates to the UEPHTN, the OHCS validates the data quality according to the standards published in the UHDD Public-Use Data File User's Manual. Information about this dataset can be found at http://stats.health.utah.gov/publications/. The UEPHTN then processes the data according to OHCS aggregation and use requirements before publishing the new data set.

      • Constraints:
        • Access: Public Health Professionals may only access Utah Tracking Network data through Secure IBIS-PH or in any other way after submitting a complete application as contained in the document Third Party Application for Access to Secure IBIS-PH for Public Health Professionals. Similarly, researchers must submit a complete application as contained in the document Third Party Application for Access to Secure IBIS-PH for Research Projects. The Scientific Review Board (SRB) does not serve as an Institutional Review Board (IRB). Applications requiring an IRB must submit for IRB approval separately. The applications can be submitted concurrently, but the SRB will not grant data access until they have received proof that the IRB has approved the research project. The SRB may conditionally approve access to Secure IBIS-PH pending IRB approval. The SRB ensures that the research proposal meets all of the data owner(s)’s requirements. Visit http://epht.health.utah.gov/epht-view/dataportal/SecureDatasetIntroduction.html for more information.
        • Use: NO-USE: This data may not be used in any way to imply Office of Health Care Statistics (OHCS) or Utah Department of Health (UDOH) endorsement of any research objective, commercial or for-profit venture; to advertise or support a commercial product; or to direct or plan targeted advertising. Data users will provide a copy of any publication draft or public presentation of this data or results derived from this data to the Utah Environmental Public Health Tracking Network (UEPHTN) which will coordinate UEPHTN and OHCS approval to publish or present. See contact information in this metadata. The OHCS requires 30 days to approve draft publications. The OHCS will provide a response in writing to the data user. RIGHT TO REFUSAL: The OHCS and/or the UEPHTN retain the right to refuse any publication or public presentation of the data or results derived from the data. ACKNOWLEDGEMENT: Use of this data requires acknowledgement of the OHCS and the UEPHTN in any publications or public presentations of the data or results derived from the data. Acknowledgement must be made that the research was supported by the OHCS with support from the Utah Department of Health. Acknowledgement must be made that the research was supported by the UEPHTN, which is partially funded by the Centers for Disease Control and Prevention. AUTHORSHIP: Authorship is required when the OHCS or the UEPHTN makes substantial contribution to the data. AUDITS: The OHCS and/or the UEPHTN retain the right to conduct on-site audits of the researcher with or without cause. Audits will be conducted after notification and during normal business hours by representatives of the OHCS or UEPHTN. The audit will observe research practices for protecting data.
